To become a member of the Lightning Spikes Track Club for the 2011 season,
please download and complete the Membership Forms which can be found in our
'Resources/Documents' area below. Athletes will not be allowed to participate
in any practices or team activities until registration fees (as noted below)
are paid and required documents are completed. The following is due at the time
of registration:

Registration fees (as noted below) must be paid in full. No exceptions.
Registration fees are not refundable or transferable.

A completed Application Form
-
A completed Parent and Athlete Profile
-
Three copies of the athlete’s birth certificate.
-
A completed Medical Authorization Form.
-
A completed Physical Examination Form signed by a licensed physician. If your
child participated in another sport this year which required medical
documentation, that documentation is acceptable; however, it cannot be more
than a year old.

Review the Team Handbook and submit a signed copy of the Acknowledgement Form
located at the end of the Handbook.

A completed Media Release Form.

All athletes will be competing in track spikes, which can be purchased from a

Athletes will begin training in track spikes on Monday, January 24, 2011.
Track spikes vary depending on events, therefore, please consult with the
Coaching Staff on the appropriate track spike to purchase.
